Basically. We all sleep. we have heavy sleepers and we got light sleepers.
We get waked up by touch , hearing. But the more chakra put into the jutsu
the harder it is to escape ( Heavy Sleepers) The less chakra put in it (Easy way to escape)
you can escape by hearing someone from the real world or by touch. but the more chakra in the jutsu the harder it may be. its basically real life.

when ur sleeping and somtimes u wake up. this is like that. in this world they are heavy sleepers. so the more chakra there is the harder it is for them to break out of the jutsu. basically its a nightmare jutsu. and you need someone esle there to break you out. or if your strong in the mind you can break out
